چکیده

A 67-year-old white man presented to Hermann Hospital upon referral from an Austin, Tex, physician for evaluation of recurrent chest pain. Eighteen months before admission the patient first noted episodes of chest discomfort, which he described as a left parasternal sensation of heaviness or tightness variably associated with moderate physical exertion. The discomfort would last 3 to 4 minutes and resolved with rest. He experienced one or two episodes each month. Two months before admission, an exercise stress test was performed and demonstrated 1.0 mm of "flat STsegment depression" in leads II, III, aVF, and V4 through V6 at peak exercise (Bruce protocol; stage IV, 10 minutes). He experienced no pain during the test, and the ECG changes resolved within 30 seconds of stopping exercise. Five days before admission, the episodes of pain increased markedly in frequency, occurred several times a day, were provoked with only minimal exertion, and occurred at rest. He also noted episodes of left arm pain, with or without chest pain, which would last 30 to 60 minutes. He was pain-free on admission. He denied any associated nausea, vomiting, diaphoresis, palpitations, dyspnea, orthopnea, edema, or fatigue. He reported no other symptoms.

Dual Antiplatelet Therapy after Coronary Artery Bypass Graft Surgery: A Review.

Coronary artery bypass graft surgery (CABG) is the gold standard treatment for relieving angina symptoms and reducing mortality among ischemic heart disease patients. As post-operative thrombosis of the grafts has been a frequent complication of CABG, antiplatelet therapy remains essential to maintain graft patency. Since a long time, aspirin has been used as a single anti-platelet agent post C...

50th anniversary historical article. Acute coronary syndromes: the degree and morphology of coronary stenoses.

In 110 patients with either stable or unstable angina, the morphology of coronary artery lesions was qualitatively assessed at angiography. Each obstruction reducing the luminal diameter of the vessel by 50% or greater was categorized into one of the following morphologic groups: concentric (symmetric narrowing); type I eccentric (asymmetric narrowing with smooth borders and a broad neck); type...

Psychosocial modulators of angina response to myocardial ischemia.

BACKGROUND Although angina is often caused by atherosclerotic obstruction of the coronary arteries, patients with similar amounts of myocardial ischemia may vary widely in their symptoms. We sought to compare clinical and psychosocial characteristics associated with more frequent angina after adjusting for the amount of inducible ischemia. METHODS AND RESULTS From 2004 to 2006, 788 consecutiv...

Effects of myocardial revascularisation in patients with effort angina and those with effort and nocturnal angina.

The effects of coronary artery bypass graft operation were studied in 32 patients with daytime ambulatory ST segment changes and 14 patients with daytime and nocturnal angina and ST segment changes. Patients had ambulatory ST segment monitoring and exercise testing before and after operation and coronary arteriography was repeated in 34 patients after operation. Before operation, patients with ...

Predictors of Physician Under-Recognition of Angina in Outpatients With Stable Coronary Artery Disease.

BACKGROUND Under-recognition of angina by physicians may result in undertreatment with revascularization or medications that could improve patients' quality of life. We sought to describe characteristics associated with under-recognition of patients' angina. METHODS AND RESULTS Patients with coronary disease from 25 US cardiology outpatient practices completed the Seattle Angina Questionnaire...

Suspected of Coronary Heart Disease

Coronary arteriographic data have been compared in 278 patients (231 males and 47 females) with the ECG response to a maximal exercise test and with the history (myocardial infarction MI, typical or atypical angina pectoris AP). The sensitivity and specificity of exercise ECG were similar in males and females. False negative ECG responses were frequent in males (40%) and false positive ECG resp...
